Medipol Mega \u00dcniversite Hastanesi Organ Nakli K\u0131sm\u0131ndan Prof. Dr. Onur Yaprak, d\u00fcnyada her y\u0131l yakla\u015f\u0131k 700 milyon insan\u0131n sivrisinek kaynakl\u0131 hastal\u0131klara yakaland\u0131\u011f\u0131n\u0131 ve y\u00fcz binlercesinin bu hastal\u0131klardan \u00f6t\u00fcr\u00fc hayat\u0131n\u0131 kaybetti\u011fine dikkati \u00e7ekerek ikazlarda bulundu.<\/p>\n
Prof. Dr. Yaprak, sivrisinek kaynakl\u0131 hastal\u0131klar\u0131n yayg\u0131n tipleri ortas\u0131nda s\u0131tma, dang, Bat\u0131 Nil vir\u00fcs\u00fc, chikungunya, sar\u0131humma ve Zika bulundu\u011funu belirterek, \u201cYap\u0131lan ara\u015ft\u0131rmalarda T\u00fcrkiye\u2019de 7 cinse ili\u015fkin toplam 62\u2019den fazla sivrisinek cinsinin oldu\u011fu saptand\u0131. Bu tiplerin ortas\u0131nda esas Aedes, Anopheles, Culex tipi bulunur. Aedes aegypti sar\u0131humma sivrisine\u011fi olarak bilinirken Aedes albopictus Asya kaplan sivrisine\u011fi olarak isimlendirilir. Farkl\u0131 sivrisinek cinsleri farkl\u0131 ate\u015fli hastal\u0131k etkenlerini ta\u015f\u0131r. Aedes cinsine ili\u015fkin 2 sivrisinek \u00e7e\u015fidi son y\u0131llarda \u00fclkemizde de g\u00f6r\u00fclmeye ba\u015flad\u0131. G\u00fcneydo\u011fu Asya’n\u0131n tropikal ve subtropikal b\u00f6lgelerine has bir cins olan Asya Kaplan Sivrisine\u011fi ve Afrika ile G\u00fcney Amerika\u2019ya mahsus bir \u00e7e\u015fit olan sar\u0131humma sivrisine\u011fi son vakitlerde \u00fclkemizde de pop\u00fclasyonu epey artan tiplerdir. Bu sivrisineklere ili\u015fkin larvalar bilhassa kullan\u0131l\u0131p at\u0131lm\u0131\u015f otomobil lastikleri, a\u011fa\u00e7 oyuntular\u0131 ve g\u00fcnlerce de\u011fi\u015ftirilmeyen a\u011fz\u0131 a\u00e7\u0131k su saklama kaplar\u0131 \u00fczere yapay kaplarda \u00fcreme g\u00f6sterir\u201d diye konu\u015ftu.<\/p>\n

Birtak\u0131m bilim adamlar\u0131 taraf\u0131ndan B\u00fcy\u00fck \u0130skender\u2019in vefat nedeni olarak g\u00f6sterilen ve sivrisineklerle bula\u015fan Bat\u0131 Nil vir\u00fcs\u00fcn\u00fcn son y\u0131llarda artt\u0131\u011f\u0131n\u0131 belirten Prof. Dr. Yaprak, \u015fu bilgileri verdi:<\/p>\n
\u201cBat\u0131 Nil vir\u00fcs\u00fc bilhassa g\u00f6\u00e7men ku\u015flar\u0131n g\u00f6\u00e7 yollar\u0131 \u00fczerinde bulunan \u00fclkelerde s\u0131k g\u00f6r\u00fcl\u00fcyor. Hastal\u0131k mevsimsel \u00f6zellik g\u00f6sterdi\u011finden \u00e7o\u011funlukla yaz boyunca ve sonbahar\u0131n birinci periyotlar\u0131nda ortaya \u00e7\u0131k\u0131yor. \u00dclkemizin de bu jenerasyonda yer almas\u0131, \u00fcstelik havalar\u0131n hala mevsim normallerinin \u00fczerinde s\u0131cak seyretmesi nedeniyle sivrisineklerin de hastal\u0131k bula\u015ft\u0131rma riski s\u00fcr\u00fcyor. Yap\u0131lan \u00e7al\u0131\u015fmalar Bat\u0131 Nil vir\u00fcs\u00fc ile enfekte olan \u015fah\u0131slar\u0131n yakla\u015f\u0131k y\u00fczde 80\u2019inin hafif ate\u015fle seyrederek g\u00fczelle\u015fti\u011fini g\u00f6steriyor. Vir\u00fcs\u00fc alm\u0131\u015f yakla\u015f\u0131k her 5 \u015fah\u0131stan 1\u2019inde ate\u015fle bir arada ishal, kusma, eklem a\u011fr\u0131lar\u0131, ba\u015f a\u011fr\u0131s\u0131, beden a\u011fr\u0131lar\u0131 yahut d\u00f6k\u00fcnt\u00fc \u00fczere ba\u015fka belirtiler geli\u015febilir. Bir\u00e7ok ki\u015fi bu tip Bat\u0131 Nil ate\u015fi hastal\u0131\u011f\u0131ndan b\u00fcsb\u00fct\u00fcn kurtulur lakin yorgunluk ve halsizlik haftalar yahut aylarca devam edebilir. Az say\u0131da bireyde \u015fiddetli belirtiler olabilir. Vir\u00fcs bula\u015fm\u0131\u015f \u015fah\u0131slar\u0131n y\u00fczde 1\u2019inden az\u0131nda ensefalit (beyin iltihab\u0131) yahut menenjit (beyin ve omurilik zar\u0131 iltihab\u0131) \u00fczere \u00f6nemli bir n\u00f6rolojik hastal\u0131k geli\u015fir ve n\u00f6rolojik tablo meydana gelen hastalar\u0131n y\u00fczde 10\u2019u hayat\u0131n\u0131 kaybeder. Prof. Dr. Onur Yaprak, sar\u0131humman\u0131n ise bir RNA vir\u00fcs\u00fc oldu\u011funa dikkati \u00e7ekerek, \u201cHastal\u0131\u011f\u0131 ta\u015f\u0131yan Aedes aegypti \u00e7e\u015fidi sivrisineklerdir. Vir\u00fcs karaci\u011fer, b\u00f6brek ve dalak \u00fczere i\u00e7 organlar\u0131 etkiliyor. Karaci\u011ferde hepatite neden olarak \u015fah\u0131sta sar\u0131l\u0131k, p\u0131ht\u0131la\u015fma fakt\u00f6rlerinde \u00fcretim eksikli\u011fi, damar i\u00e7i p\u0131ht\u0131la\u015fma, kanama \u00fczere meselelere yol a\u00e7ar ve nihayetinde \u015fok tablosu geli\u015fir. Vir\u00fcse y\u00f6nelik bir a\u015f\u0131 olup endemik Afrika b\u00f6lgelerine seyahatler s\u0131ras\u0131nda a\u015f\u0131 kullan\u0131l\u0131yor. Hastal\u0131\u011f\u0131n 3 evresi var. Birinci evrede ate\u015f, bulant\u0131, kusma, kas a\u011fr\u0131lar\u0131 g\u00f6r\u00fcl\u00fcr. Birka\u00e7 g\u00fcn s\u00fcren ve \u00f6b\u00fcr enfeksiyonlardan ay\u0131rt edilemeyen tablonun akabinde \u015fikayetlerin azald\u0131\u011f\u0131 ikinci devir ba\u015flar. Fakat hastalar\u0131n y\u00fczde 15\u2019i birinci 3-6 g\u00fcn sonra kan zehirlenmesi olarak bilinen, karaci\u011fer iltihab\u0131n\u0131n sar\u0131l\u0131\u011f\u0131n ve b\u00f6brek yetmezli\u011finin ba\u015flad\u0131\u011f\u0131 \u00fc\u00e7\u00fcnc\u00fc periyoda girer. Bu devirde hastalarda t\u0131pk\u0131 koronavir\u00fcste oldu\u011fu \u00fczere \u00e7ok sitokin f\u0131rt\u0131nas\u0131 ile birlikte yayg\u0131n damar i\u00e7i p\u0131ht\u0131la\u015fma sorunu geli\u015fir, cilt, burun ve ba\u011f\u0131rsaklar\u0131ndan kanama ba\u015flar. Sonu\u00e7ta hasta \u015fok ve komaya, kalp, akci\u011fer ve beynin etkilendi\u011fi \u00e7oklu organ yetmezli\u011fine girer. D\u00fcnya \u00e7ap\u0131nda her y\u0131l yakla\u015f\u0131k 30 bin ki\u015fi sar\u0131hummadan hayat\u0131n\u0131 kaybediyor\u201d dedi.<\/p>\n
KEM\u0130K A\u011eRISI ENFEKS\u0130YON BEL\u0130RT\u0130S\u0130<\/b><\/p>\n
Dang ate\u015finin de Aedes cinsi sivrisineklerle bula\u015ft\u0131\u011f\u0131na de\u011finen Prof. Dr. Yaprak, \u015fu de\u011ferlendirmede bulundu:<\/p>\n
\u201cK\u0131r\u0131lm\u0131\u015f \u00fczere kemik a\u011fr\u0131lar\u0131na neden oldu\u011fu i\u00e7in \u2018k\u0131r\u0131k kemik hummas\u0131\u2019 olarak da bilinir. Hastal\u0131k genelde 1 hafta s\u00fcrer ve d\u00fczg\u00fcnle\u015fme ile tamamlan\u0131r fakat her 20 \u015fah\u0131stan 1\u2019i \u015fiddetli dang hummas\u0131 tablosuna girer, hatta mevte sebebiyet verecek kadar genel durumu berbatla\u015fabilir. Dang ile enfekte olan her d\u00f6rt \u015fah\u0131stan biri hastalan\u0131r. Semptomlar hafif yahut \u015fiddetli olabilir. \u015eiddetli dang hummas\u0131 birka\u00e7 saat i\u00e7inde hayat\u0131 tehdit edebilir ve \u00e7oklukla hastaneye yatmay\u0131 gerektirir. Hafif dang hummas\u0131 semptomlar\u0131, ate\u015f, a\u011fr\u0131 ve s\u0131z\u0131 yahut d\u00f6k\u00fcnt\u00fcye neden olan \u00f6b\u00fcr hastal\u0131klarla kar\u0131\u015ft\u0131r\u0131labilir. Sar\u0131humma da oldu\u011fu \u00fczere Dang hummas\u0131nda karaci\u011fer tutulumu g\u00f6r\u00fclebilir. D\u00fcnyada her y\u0131l iddias\u0131 100 ila 400 milyon ki\u015fi bu enfeksiyondan etkilenir. Her y\u0131l d\u00fcnya \u00e7ap\u0131nda yakla\u015f\u0131k 25 bin ki\u015fi dang hummas\u0131ndan \u00f6lmektedir. Bu tip vir\u00fcslerden korunmak i\u00e7in sakin sular\u0131 \u0131slah etmemiz ve sivrisinek denetimi yapmam\u0131z gerekiyor. Sokak hayvanlar\u0131 i\u00e7in d\u0131\u015far\u0131ya b\u0131rak\u0131lan sular\u0131n birka\u00e7 g\u00fcnde bir de\u011fi\u015ftirilmesi gerekmektedir. \u0130la\u00e7laman\u0131n vaktinde yap\u0131lmas\u0131, sivrisineklerin a\u011f\u0131r oldu\u011fu b\u00f6lgelerde uzun kollu giysiler giyilmesi ve sinek kovucu sprey \u00fczere kollay\u0131c\u0131 tedbirlerin al\u0131nmas\u0131 korunmada katk\u0131 sa\u011flar. Pencerelere sineklik tak\u0131lmas\u0131, kap\u0131 ve pencerelerin a\u00e7\u0131k b\u0131rak\u0131lmamas\u0131 \u00f6teki tedbirlerdendir. \u0130la\u00e7lama yap\u0131lamayacak tatl\u0131 su kaynaklar\u0131nda ise sivrisinek larvas\u0131 yiyen bal\u0131klar \u00fczere canl\u0131lardan faydalan\u0131lmas\u0131 gerekir.\u201d<\/p>\n<\/p>\n
